How the verdict is decided: grounding metadata arrives with one entry per source, and the URLs in it are Google redirect links rather than the publisher's own. The probe follows each redirect and matches the FINAL host against your domain, so a site is credited under its own name instead of under Google's redirector. A redirect that cannot be followed keeps the redirect host and is still listed, rather than being dropped.
Grounding is a setting, not a promise about coverage. Gemini decides how much to ground a given answer on, and a short reply can rest on very few sources. A small list is a fact about that answer rather than a sign something went wrong — when something does go wrong you get Unavailable, which says so in as many words.
